Healthcare Documentation & Billing Readiness

Turning Complex Clinical Documents into Billing-Ready Documentation with AI

Turns clinical PDFs into validated, billing-ready documentation packages, cutting preparation time from about 20 minutes to under 5 per patient.

Company Context

RPM Provider from Atlanta

Built With

Multimodal LLM-powered document extraction, API integration with the RPM platform, and deterministic validation rules kept separate from AI interpretation.

The Challenge

Remote patient monitoring depends on documentation spread across clinical documents and the RPM platform: physician orders, consent forms, device setup records, clinical notes, monitoring reports, and treatment-management records, often scanned, multi-page, or built around dense tables. Before physicians and their staff could prepare claims, someone had to find the relevant information, match each document to the right patient and service period, and spot missing or inconsistent evidence by hand. The work consumed significant staff time and delayed complete documentation from reaching billing review.

Before and After

One billing cycle for the RPM documentation team

Clinical documentation is organized and checked as it arrives, so month-end starts with packages ready for review.

Before · without Apinso

Scanned orders, consent forms, and device records are read and filed by hand.

After · with Apinso

Every document is automatically read, matched to the right patient, and linked to its source.

Billing-ready documentation for every patient and service period, with staff focused only on the exceptions.

What We Built

A document intelligence and workflow system that connects unstructured clinical documents with structured data in the RPM platform: Clinical PDFs & RPM Data → AI Document Intelligence → Reconciliation & Validation → Exception Resolution → Billing-Ready Documentation. AI and deterministic rules each do the job they're suited to: AI reads and interprets the documents, while configurable rules decide whether the documentation meets each explicit requirement. The system prepares documentation for billing review; physicians and their staff review it and submit claims through their existing billing process.

Key Capabilities

Reads complex clinical PDFs, including scanned, multi-page, and table-heavy records, using multimodal LLMs that interpret page layout and tables, not just text

Classifies each document and extracts patient identifiers, provider details, dates, and supporting evidence, linked back to the source page

Reconciles extracted information with patient, enrollment, device, and monitoring records retrieved through the RPM platform's APIs

Checks documentation completeness with deterministic rules, distinguishing a document that is present from one that actually provides the required evidence

Flags missing documents, inconsistent dates, and possible patient mismatches for staff to resolve, rather than resolving them silently

Assembles a documentation package for each patient and service period, and updates the RPM platform with document references, check results, and outstanding actions

The Impact

A 60% reduction in manual effort to extract, reconcile, and assemble supporting documentation.

Documentation preparation reduced from about 20 minutes to under 5 minutes per patient each service period.

Physicians and their staff review every documentation package and remain responsible for submitting claims. Passing the documentation checks means a case is ready for human billing review; it doesn't determine claim eligibility, guarantee reimbursement, or replace clinical judgment.

Enterprise Controls

Clear division between AI and deterministic logic: AI interprets documents and extracts information, while configurable, deterministic rules decide whether each documentation requirement is met

Human review for ambiguous patient matches, conflicting clinical information, and low-confidence extractions

Evaluation against a labeled set of reviewed documents, measuring precision, recall, and F1 for document classification and for each extracted field

Ongoing monitoring of extraction quality in production, using staff corrections and exception rates to catch drift by document type

Source traceability: every extracted value links back to the document and page it came from

Full audit trail across extraction, validation checks, exceptions, resolutions, and platform updates

Operates within the RPM provider's cloud-hosted, HIPAA-governed environment, accessing patient information through the platform's authorized APIs
